Use this Excel spreadsheet to audit each of your areas applying the principles of 5S. When you complete the scores for each of the 40 criteria, the spreadsheet automatically calculates the average for that 'S' and updates the corresponding chart.

5S & 6S Cleaning Schedules
These simple forms will help your team to sustain their improvements by maintaining a high level of cleanliness in their work area. Use the Initial Cleaning Plan to assist your team in first creating a high standard of cleanliness during the "Shine" portion of their 5S or 6S activity. Then use the 5S / 6S Sustain Activity Schedule to coordinate which team member will be responsible for maintaining the cleanliness of specific areas and at what schedule.

Use this Excel spreadsheet to track the progress of your value stream transformation. Not only does this spreadsheet allow you to very visually show the progress of your transformation, but it also gives a step-by-step list of activities necessary to transform any production value stream into a world-class example. Each rating is clearly defined using embedded notes so there will be little variance between auditors.

Value Stream Assessment Standard Work
This file includes a list of the ranking criteria that can be posted beside the assessment results to indicate the criteria for each score sot that team members know what they need to focus on to improve their results. The standard work also indicates how often an assessment should be completed, and indicates what each color means.
